COVID-19 UPDATE

March 11, 2022

As per recent provincial health orders, masks are now optional for visitors as well as staff.  We ask all visitors to please be respectful of each individual’s choice during this transition time.  Office hours are still 8:00 a.m. to 4:30 p.m.  You will note barriers are still in place at the front desk which will be maintained for the time being.

January, 2022

During this Omicron wave, we are doing our best to maintain normal staffing of our office during key business times.  Normal office hours for in-person visits are Monday to Friday from 8:00 a.m. to 4:30 p.m. however, our team is small and situations may arise where the office may not be able to accommodate these hours. Before visiting our office in person, we recommend calling to verify there are staff members present.  We recommend faxing, mailing, or emailing your claims and documents into the office during this time versus in-person visits.

January and February are extremely busy times for member claim processing.  Due to this and the Omicron impacts, processing times may be longer than normal.  We appreciate your patience as our team works to turnaround member claims.

Note that the deadline for submission of claims with any date of service (and proof of payment) in 2021 is March 31, 2022.  Please ensure to submit your 2021 dental, optical hearing aid and orthodontic claims to our office prior to this deadline.

November 18, 2021

The pension and benefits office is open for in-person member visits Monday to Friday from 8:00 am to 4:30 pm.  At this time, we are requesting a maximum of two (2) visitors in the office at one time.  For the continued safety of our staff and until health orders allow, we are requesting these limits be maintained as well as social distancing rules.  We are also requesting visitors to wear masks/facial coverings.

We sincerely thank our members for your support during the past year and a half and hope everyone and their families have remained safe and healthy.

A friendly reminder the deadline to submit 2021 dental, optical and hearing aid claims is March 31, 2022. 


Dental Claim Requirements

Payment will be made on presentation of the following:

  • An Original Paid Receipt and
  • An Original Standard Dental Claim Form which shows:
    • the date of service,
    • amount of payment
    • name of member (or dependent)
    • If there is another plan involved with a claim (i.e. spouse’s plan), members must provide copies of this coverage along with the above originals.

Payment for dental can be made directly to your dentist.  Your dentist must call the Welfare Plan office to have this arrangement set up.

Notes:

  • Unnecessary or cosmetic dentistry, including bleaching and veneers, are not covered by this Plan except with the consent of the Plan Trustees.
  • Any dental expenses resulting from an accidental injury are covered under your extended heath plan, please check your Pacific Blue Cross brochure for details.

Please be advised that the Plan Trustees have the ability to pro-rate coverage. 

**Members with less than four (4) months of coverage in the calendar year will be pro-rated to 1/12 of the dental limit for each month of coverage.

Hearing Aids Claim Requirements

Payment will be made on presentation of the following:

  • An Original Itemized Paid Receipt indicating:
    • who the hearing aid was purchased for
    • the method of payment

Note: Hearing aids for dependent children under twenty-one years of age are covered under your extended health plan, please check your Pacific Blue Cross brochure for details.

Submitting Dental and Hearing Benefit Claims

The benefits office will now accept claims by mail, e-mail, or fax for optical. Please keep your original receipts in case we ask to see them.

The following documents and information are required for reimbursement: 

  1. Original Itemized Paid Receipt
  2. Standard Dental Claim Form (dental only)
  3. Member’s Name
  4. Member’s Address (full Street Address, City, Province, and Postal Code)

Please send or ask the dental office or hearing aids vendor to send PDF, JPEG or fax copies of the required documents.

Alternatively, we have direct reimbursement available for dental offices. To take advantage of direct reimbursement, the dental office should contact the benefits office to get enrolled.

Optical Claim Requirements

  • Expenses for corrective laser eye surgery, cataract lenses and eye examinations that are not covered by the BC Medical Plan are allowed.
  • Eyeglasses/contact lenses require a prescription for reimbursement. Credit card, cash or debit purchases require a register receipt to validate payment.
  • Purchases of frames only are not eligible for reimbursement.
  • Claim forms are required as of January 1, 2022 and must be completed in full by the member and returned via hardcopy, email or fax to the Welfare Plan office along with:
    • An itemized paid receipt. Handwritten receipts are not accepted.
    • An official optical (from a licensed optometrist) prescription. Prescriptions must not be less than 24 months old from the date of service
    • Claim forms are available from the Welfare Plan office or can be downloaded from the Forms and Library section on the website.

Submitting Optical Benefit Claims

The benefits office will now accept claims by mail, e-mail, or fax for optical. Please keep your original receipts in case we ask to see them.

The following documents and information are required for reimbursement:

  1. Optical Claim Form (effective January 1, 2022)
  2. Itemized Original Paid Receipt
  3. Method of Payment – cashier till slip, Interac receipt, credit card receipt, or redacted banking statement
  4. Prescription (must be within two (2) years from the date of purchase)
  5. Member’s Name
  6. Member’s Address (Full Street Address, City, Province, and Postal Code)
